Home Health Nurse

<p>Pride Health is hiring an <strong>RN Home Health</strong> to support our client’s medical facility based in <strong>Milwaukee, WI 53226</strong>. This is a <strong>13-week </strong>assignment with a contract-to-hire opportunity and a great way to start working with a top-tier healthcare organization!</p><p><br></p><p><strong>Job Title: RN Home Health</strong></p><p><strong>Location: Milwaukee, WI 53226</strong></p><p><strong>Weekly blended pay: $1,900.00/week - $2,200.00/week</strong></p><p><strong>Shift: Monday–Friday, 8:00 AM–5:00 PM, 40 hours/week; occasional on-call required.</strong></p><p><br></p><p><strong>Patient Load: </strong>6–8 Medicare Home Health visits per day; Homecare Homebase (HCHB) experience preferred.</p><p><strong>Responsibilities: </strong>Provide skilled home health nursing, complete assessments (including OASIS), develop and update care plans, administer treatments, educate patients/families, and coordinate care with physicians and the interdisciplinary team.</p><p><strong>Clinical Duties:</strong> Bedside nursing, symptom management, crisis intervention, family education, palliative/end-of-life care, documentation, and supervision of LPNs/LVNs and home health aides.</p><p><strong>Requirements: </strong>Minimum 2 years of RN experience (required for PASSPORT patients), active RN license, valid driver's license, automobile insurance, and tablet proficiency.</p><p><strong>Key Skills:</strong> Patient care management, bedside nursing, symptom management, family intervention, interdisciplinary collaboration, communication, problem-solving, and decision-making.</p><p><br></p><p>Pride Global offers eligible employee’s comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance, and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance, and employee discounts with preferred vendors.</p><p><br></p><p><strong>Equal Employment Opportunity</strong></p><p>Pride Health is an equal opportunity employer.
